Services

Supplemental Medical Transportation Assistance Program - provides rides for persons with medical treatments which are frequent, or of long duration, or more than 25 miles from the patient's home. Riders receive assistance with transportation costs not covered by Medical Assistance. Co-pay is based on ability to pay.

Common Carrier Medical Assistance Transportation - is available to persons eligible for Title IX Medical Assistance (MA) who do not have Specialized Medical Vehicle (SMV) eligibility certification. Rides are available to MA-covered services. Transportation may be provided by bus, cab, SMV, or other common carrier. This is an individualized service. Rides are coordinated through HMOs, approved clinics and agencies, social or caseworkers, or DCDHS staff. Service area includes all of Dane County and may cover pre-approved out-of-county travel. There is currently no donation, fare or passenger co-pay.
