Dane County Department of Human Services manages the Community Options Program for individuals with severe physical disabilities that would qualify a person for a nursing home level of care. The Department also purchases limited services for persons with a physical disability through Access to Independence and Community Living Alliance.

The following links provide information on selected physical disabilities, as well as, information on how to help.

Arthritis and Related Diseases - Arthritis is actually an umbrella term used for a group of more than 100 medical conditions. The Arthritis Foundation web site provides facts on arthritis, the different forms, and management principles.

Chronic Obstructive Pulmonary Disease - This American Lung Association web site provides information on the prevalence, diagnosis, and treatment of chronic obstructive pulmonary disease.

Multiple Sclerosis - The National Multiple Sclerosis Society web site provides information on the symptoms, diagnosis, causes, and treatments related to multiple sclerosis.

Neuromuscular Diseases - This Muscular Dystrophy Association web site provides information on the characteristics, age of onset, progression, and other information related to muscular dystrophy and related diseases.

Spinal Cord Injury - This State of Wisconsin web site provides information regarding the prevalence, causes, and costs of spinal cord injury in Wisconsin. There is also a link to the National Spinal Cord Injury Association.

Stroke - The National Stroke Association web site provides information on stroke prevention, risk factors, symptoms, types, and recovery.

Services are typically provided to:

  • Dane County residents
  • Age 18 to 64
  • Who have a significant physical disability
  • Meet functional eligibility criteria
  • Meet financial eligibility criteria

The Dane County Department of Human Services - STEP Unit assists adults with physical disabilities in applying for the Community Options Program, Food Share, and Medical Assistance throughout Dane County. Income and assets are taken into account and co-pay may be required. Please call (608) 242-7400 for more information.

The Dane County Department of Human Services purchases limited services for persons with a physical disability through Community Living Alliance. Services are also provided based on the need of the consumer through the Community Options Program. A description of some of these services follows.

Community Options Program (COP) - The Community Options Program delivers community-based services to Wisconsin citizens who need long term assistance in performing daily living activities in their own homes or apartments. Any person regardless of age who, due to chronic disabilities, needs a nursing home level of care which Medical Assistance would purchase is eligible for Community Options Program of Medicaid Waiver programs. Based on individual service plans, a variety of services can be funded including, but not limited to, nursing and other in home care, home modifications, case management, adaptive equipment, transportation, residential care, vocational services, respite care, and day support services. Depending on an individual's income and assets, the individual may be required to contribute toward the cost of care.

Medical Assistance Personal Care Program - Dane County Department of Human Services also offers the Medical Assistance Personal Care (MAPC) program to provide personal care services to Medical Assistance consumers who choose this option. MAPC services include, bathing and dressing, as well as some light housekeeping that is directly related to the consumers' care. Personal Care services are supervised by a registered nurse.

Some services may require a co-pay.

Access to Independence - serves as an Independent Living Center which is a consumer-directed, non-profit organization that provides an array of services including: peer support, information and referral, independent living skills training, advocacy, community education, personal care, and service coordination. Services are provided without cost to persons who are blind, visually impaired, deaf, hard of hearing, and have a severe physical disability.

Social Security Disability - This web site provides information on eligibility and applying for social security disability benefits.

Supplemental Security Income - This web site provides information on state supplemental security income benefits, as well as, links to the federal SSI benefits information.

Vocational Rehabilitation, Wisconsin Department of Workforce Development - The DVR web site provides information on employment services for persons with a disability.

Wheelchair Recycling Program - This web site provides a listing of lower cost wheelchairs, power wheelchairs, crutches, walkers, bath stools, commodes, grab bars, raised toilet seats, and overbed tables.

Wisconsin Council on Physical Disabilities - The Council was created by the state legislature in 1989. Its mission, established by state statute 15.197 (4), is: to develop and implement a state plan for services to people with physical disabilities; to advise and make recommendations to state agencies on relevant legislation; to promote public awareness about the abilities of and barriers to people with physical disabilities; to encourage the development of programs and policies that prevent physical disabilities; and to submit recommendations in an annual report to the state legislature.

WisTech Assistive Technology Services - WisTech provides information on selecting, funding, installing and using assistive technology. Assistive technology (AT) is any product or device that enables people with disabilities to improve their independence and quality of life. These items include commercially-made equipment, custom-made devices or common products that have been modified to serve a specific need.
