Description
Dane County Department of Human Services, through its Area Agency on Aging, Public
Health Madison Dane County, and Adult Community Services Division coordinates and
purchases an array of services designed to improve the health, safety, and well-being
of older adults in Dane County and to enable them to reside in their own homes and
apartments as long as possible.
Area Agency on Aging
Pursuant to the Federal Older Americans Act, the Wisconsin Elders Act and in cooperation
with the Area Agency on Aging Board, staff provide and purchase the following services:
information and assistance, elder abuse and neglect investigation, nutrition, outreach,
case management, transportation, benefit specialist, volunteer opportunities, home
care/chore services; develop and implement programs and services to meet the needs
of caregivers of elders and for older persons who are the primary caregivers of
minor family members, conduct an ongoing assessment of service system capacity and
gaps; develop a three year County Aging Plan including initiatives consistent with
identified needs and gaps; coordinate services offered by Dane County and community
agencies.

Services
|
|
Benefit Specialist - The Area Agency on Aging of Dane County provides
funding for Benefit Specialist services that provide assistance to elders in understanding
and accessing government benefit programs (Medicare, Medical Assistance, Social
Security, SSI, Food Stamps, and many others).

Caregiver
Support Services - The Area Agency on Aging of Dane County coordinates
the National Family Caregiver Support Program. Services include: case management,
support groups for caregivers, training, supportive home care services, short-term
respite care, resource information, and more.

Case Management - Case management services
are provided to adult residents of Dane County, age 60 and older, who live in their
own homes or apartments or the home of a family member. The goal is to provide older
adults the support they need to remain safe in their own homes and community by
identifying resources, coordinating supportive services, developing emergency plans,
and conducting home safety checks. Services are coordinated through agencies serving
geographically defined focal point areas.

Community Options Program (COP) - The Community Options Program
delivers community-based services to income eligible citizens who, due to chronic
disabilities, need long term assistance in performing daily living activities in
their own homes or apartments. A nursing home level of care is required for the
Community Options Program and Medicaid Waiver programs. Based on individual service
plans, a variety of services can be funded including, but not limited to, nursing
and other in home care, home modifications, case management, adaptive equipment,
transportation, residential care, vocational services, respite care, and day support
services. Depending on an individual's income and assets, the individual may be
required to contribute toward the cost of care.

Elder Abuse - The Elder Abuse/Neglect Unit of the Area Agency on
Aging of Dane County investigates reports of material abuse, physical abuse, neglect
and self neglect of persons age 60 and older. The elder abuse team links older adults
with additional resources that may be of assistance. All reports and records are
confidential.

Older Worker Program
- WISE Options older worker program offers paid training for employment opportunities
for older adults age 55 and older.

for adults 55 and over.

Senior Nutrition Program - The Area Agency
on Aging of Dane County purchases congregate and home delivered meal programs for
adults age 60 and older in accordance with the Older Americans Act and the Wisconsin
Elderly Nutrition Program Manual of Policies and Procedures. Congregate meals are
provided at 31 sites throughout Dane County - advance reservations are required.
Home delivered meals are available to frail, homebound Dane County adults age 60
and older who have no other source for their noon meal and who meet additional requirements.
The goals are to provide nutritious congregate meals coupled with socialization
and educational or entertainment oriented programming to older adults who make reservations
for site-based meals and to provide nutritious, home-delivered meals to homebound
older adults so that they can remain in their own homes and communities.

Supportive Home Care - The Area Agency on
Aging of Dane County purchases supportive home care services for older adults who,
due to physical and/or mental impairments are not capable of full-time independent
living. Services may include home care; household management such as home chore,
grocery shopping or other chore assistance; yard work and shoveling; and respite
and/or home chore assistance for caregivers. Services are coordinated through agencies
serving selected focal point areas.

Transportation Services
- Group and individual transportation services which enable seniors to remain independent
and in their own homes are provided. Rides are provided to medical appointments,
congregate meal sites (nutrition sites), community/senior centers, grocery and general
shopping, and other services.

Cost
|
|
Cost will vary depending on the service. Some services may be free while others
require a co-pay or payment based on a sliding scale fee. While there is no charge
for congregate or home delivered meals, participants are asked to donate what they
can reasonably afford.
