Description
The full range of childhood immunizations, as well as, some adult immunizations are
provided at no charge on a regular basis at Public Health Clinic locations throughout
Dane County. Vaccines are available to prevent many diseases that can have serious
complications and may even be fatal. Vaccines are available for
measles,
mumps,
rubella,
polio,
diphtheria,
tetanus,
pertussis,
HiB disease,
hepatitis B, and
varicella (chickenpox),
pneumococcal disease,
meningococcal disease,
influenza, and
hepatitis A.

General Eligibility

All Dane County residents are eligible for immunizations. Although there is no charge for most vaccines, some restrictions and costs may apply to some vaccines. Children under 18 years of age must be accompanied by a parent or legal guardian.

How to Apply

Immunizations are provided at Public Health Clinic locations throughout Dane County.
- No appointments are needed.
- Bring all immunization records to each clinic.
- Persons in need of an interpreter or accommodations for a disability should call (608-266-4821) in advance.

Immunize on Schedule
|  |
Immunizing your children is one of the best things you can do to keep them healthy. Since childhood diseases are especially dangerous for infants, it is important to start your child's immunizations on time, usually at birth or 2 months. Children need 80% of their vaccinations in the first two years of their lives.

Keep Immunization Records

Parents should keep up-to-date immunization records in the home even though the doctors and public health clinics also keep them. All states have immunization requirements for school entry, so personal immunization records must be presented whenever children register for school. The Wisconsin Immunization Record is available from the Dane County Division of Public Health, your doctor, or HMO.

Wisconsin Immunization Law

Wisconsin's Immunization Law requires children in licensed day care centers and students through grade 12 to be immunized against certain diseases according to their age or grade level. Students must provide dates (month, day, and year) of immunization upon admission to school or be subject to exclusion or legal action. A fine of up to $25 per day may be imposed for non-compliance with the law. Waivers are available for medical, religious, or personal conviction reasons.
