Description
With the goals of assuring access to health care and improving the health of the community as a whole, the Dane
County Department of Human Services provides access to a range of medical assistance services.

Generally services are available to Dane County residents. See each service for additional information and
links to sites that offer information regarding specific eligibility criteria.

Services
|
|
BadgerCare Plus
- Since February 1, 2008, all kids in Wisconsin have been able to get the health care they need at a
price their families can afford through BadgerCare Plus. BadgerCare Plus allows every Wisconsin family,
regardless of income, to have access to affordable, comprehensive health care coverage for their kids. In
addition, BadgerCare Plus also offers coverage for more pregnant women, self-employed parents and farmers, and
youth aging out of foster care. Low-income families can enroll their children into BadgerCare Plus at no charge.
Co-payments for health care services range from $.50 to $3.00 for families with lower incomes. Families with
higher incomes will be required to pay reasonable monthly premiums and co-payments for certain services.
Co-payments for health care services range from $5.00 for prescription drugs and $100.00 for a hospital stay.
Under both plans, there are no co-payments for well child checkups and other preventive services. For more

Health Check - Health
Check is a preventive health check-up program for anyone under the age of 21 who is currently eligible for
Wisconsin Medicaid or BadgerCare. Health Check provides a head-to-toe medical exam, immunizations, eye exam,
lab tests, growth and development check, hearing check, nutrition check, teen pregnancy services, and more.
Assistance is provided with transportation if needed. For more information on Health Check, see the State web

Immunizations - The full range of childhood immunizations, as well some
adult immunizations, are provided on a regular basis at Public Health Clinic locations throughout Dane County.
Vaccines are available to prevent many diseases that have serious complications and may even be fatal. Examples
of some vaccines: measles, mumps, rubella, polio, diphtheria, tetanus, pertussis, Hib disease, hepatitis B, and
varicella (chickenpox). Although there is no charge for most vaccines, some restrictions and costs may apply

Medicaid -
Wisconsin Medicaid is a joint federal/state program that pays for medical services for people with disabilities,
people 65 years of age and older, children and their caretakers, and pregnant women who meet the program's

assistance may be processed through the mail. Appointments may be requested but are not required.
Ombudsman - Dane County has an Ombudsman available to answer questions
regarding eligibility and accessing Medicaid and other publicly funded health care services.

SeniorCare -
SeniorCare is Wisconsin's prescription drug assistance program for Wisconsin residents who are 65 years of age
or older who meet eligibility requirements. All participants must pay a nominal annual enrollment fee. All
seniors are subject to certain out-of-pocket expenses depending on their annual income. For more information on

Wisconsin Well
Woman Program (WWWP) - WWWP is a statewide program that provides free or low cost health screenings,
diagnostic tests and referrals related to heart disease, breast cancer, cervical cancer, diabetes, high blood
pressure, osteoporosis, depression, and domestic abuse for eligible women. Program enrollment is completed over
the phone and an enrollment packet is mailed to participants with instructions about how to schedule their
clinic appointments. Transportation to and from appointments is also available. All materials are available in
Spanish.

Cost

While there is no charge for most services, some do require an enrollment fee or have other costs based on a
sliding fee scale. These are indicated in the Services section where appropriate.

2005 Community Resource Guide - This guide offers local Dane
