Description
The Dane County Department of Human Services purchases an array of services to meet
the needs of consumers, to advance prevention efforts, and to provide specialized
administrative and facility support.

Types of Providers

Adult Family Homes
Adult Family Homes are places where 1-4 adults reside and receive care or services
that are above the level of room and board and may include up to 7 hours per week
of nursing care. Adult Family Homes serve persons with advanced age, dementia, mental
health issues, developmental disabilities, traumatic brain injury, AIDS, alcohol
and other drug abuse, correctional clients, and/or persons with a terminal illness.
The Dane County Department of Human Services certifies homes where one or two persons
reside while the State licenses homes where three or four people reside. Certificates
are issued for one year and may be renewed provided all standards continue to be
met.
Family Foster Care
Dane County foster parents provide a home to children and youth who are unable to
live with their families due to issues of abuse, neglect, or other child welfare
concerns. A foster parent works with the Department and the child's family to help
achieve a safe and permanent living situation for a child, hopefully with birth
family or other relatives whenever possible.
Purchase of Service (POS) Contracts
Dane County Department of Human Services puts out requests for proposals (RFPs)
on a five year cycle for the majority of its purchased services.RFPs are typically
released at the end of the first week in May for contracts beginning the following
year. RFPs for new or specialized services may be released at other times throughout
the year. Persons interested in bidding should also check out the vendor registration
process and living wage information.

Living Wage Information
Section 25.015 of the Dane County Ordinances requires that persons who are employed
by the County, employed directly in work performed as a result of a service contract
with the County or employed by an employer who is the beneficiary of economic development
assistance from the County be paid no less than a specified hourly wage known as
the living wage. For more information on the living wage, please check the
Dane County Purchasing Division's web site.
Vendor Registration
Per Dane County Ordinance, Section 62.15, "Any person desiring to bid on any county
contract must register with the purchasing manager and pay an annual registration
fee of $20." Payment may be made via credit card on-line or by check in the mail
or in person at the Purchasing Division office. For more information on vendor registration,

Boilerplate Contracts
|

The current boilerplate contracts are available in Adobe Acrobat which requires
the free, downloadable Adobe
Acrobat Reader. The contracts include information on civil rights compliance,
affirmative action, Americans with Disability Act, living wage, confidentiality,
client grievance, insurance, fiscal, reporting, and other requirements.
Substitute Care - This is
the boilerplate contract for residential care centers (RCCs), treatment foster care
providers, and group home providers..
Community Based Residential Facilities
(CBRF) and Residential Care Apartment Complexes (RCAC) - This is the boilerplate
contract for community based residential facilities and residential care apartment
complexes, including a separate Schedule A for each of those services:
Community Services Block Grant Program Contract - this is the boilerplate contract for projects funded under the Community Services Block Grant program. This boilerplate is for projects which fall under the Federal Fair Labor Standards. Non-construction projects would be exempted from the Fair Labor Standards.
Self-Directed Support Contract
- This is the boilerplate contract for agencies providing support and services through
the Self-Directed Support System. Self-directed supports are community based services
to adults with developmental disabilities, designed to allow the individual to live
and work in the most integrative environment possible, while providing an appropriate
amount of oversight to ensure the person's health and safety. A person's services
are based on their individual support plan that is developed in conjunction with
their support team. Self-directed supports are designed to build, strengthen or
maintain informal networks of community support for the individual.
Standard Purchase of Service Contract
- This is the standard boilerplate contract for all other services provided through
the Department. A short form
version is also available for program services costing less than $10,000
and for contracts for training, evaluation, and administrative services.
