If an AODA service provider is required to submit monthly AODA Client Service reports (610 reports), they must also submit AODA Episode reports. These reports are used to capture qualitative information required by the State on persons receiving publicly funded AODA services.
Data is collected at the time the client is first served. The forms may be mailed or faxed to the Department by the 10th of the following month. If mailed, please send them to:
