Client Data Collection and Reporting

Most Purchase of Service agencies are required to submit client registration and monthly client service reports to the Department. Additionally, AODA and Mental Health providers must submit periodic client episode reports. This data enables the Department's program managers to evaluate service delivery and program performance.

The Department must submit the service and episode data to the State in order to meet program funding requirements, and the State determines the specific requirements of the service and episode reports. The State's data management system is commonly known as HSRS (short for the Human Services Reporting System). The HSRS website External link specifies the data required by the State, and in turn by the Department, although the Department collects it in different formats. Much of the State and federal funding received by the Department is determined, directly or indirectly, by what the Department reports to HSRS.

The Department uses a variety of data collection systems including direct online data entry, electronic file submission and paper forms. The Department is gradually moving to more automated methods of collecting data.

There are a variety of reports that are required for a variety of conditions.
