Boilerplate Contracts
The current boilerplate contracts are available in Adobe Acrobat which requires the free, downloadable Adobe Acrobat Reader. The contracts include information on civil rights compliance, affirmative action, Americans with Disability Act, living wage, confidentiality, client grievance, insurance, fiscal, reporting, and other requirements.
This is the boilerplate contract for residential care centers (RCC), treatment foster care providers, and group home providers.
This is the boilerplate contract for community based residential facilities and residential care apartment complexes, including a separate Schedule A for each of those services:
This is the boilerplate contract for projects funded under the Community Services Block Grant program. This boilerplate is for projects which fall under the Federal Fair Labor Standards. Non-construction projects would be exempted from the Fair Labor Standards.
This is the boilerplate contract for agencies providing support and services through the Self-Directed Support System. Self-directed supports are community based services to adults with developmental disabilities, designed to allow the individual to live and work in the most integrative environment possible, while providing an appropriate amount of oversight to ensure the person's health and safety. A person's services are based on their individual support plan that is developed in conjunction with their support team. Self-directed supports are designed to build, strengthen or maintain informal networks of community support for the individual.
This is the standard boilerplate contract for all other services provided through the Department. A short form version
is also available for program services costing less than $10,000 and for contracts for training, evaluation, and administrative services.
