ADRC Proposal Outline

Below is the outline that counties must follow in preparing an application for an Aging and Disability Resource Center.Each component in the outline must be addressed. This is similar to the outline used in Dane County's 2007 draft ADRC application. Currently Dane County Department of Human Services staff are working their way through the outline, section by section, updating information and providing additional detail as necessary. Dane County will post sections of the draft application as they are completed. Sections A.1 through A.10 are posted below.
