Case Management
Fifteen agencies located throughout Dane County have primary responsibility for programs and services for older adults (age 60 and over). Serving designated geographic areas, these focal point agencies provide the entry points through which older persons – and their families – access a broad array of programs and services, including nutrition and case management.

Case Managers are located in all fifteen agencies. These are trained professionals who have expertise in working with older adults and their families. The primary goal for a Case Manager is to provide older adults with the support, resources, and community services that they need to safely remain in their own homes and communities.
Case Managers conduct home visits to assess the individual needs of each older adult and then provide information, referrals, and advocacy according to those needs.
Some individuals are looking for short-term information, while others benefit from ongoing case management support. Case Management services are provided on a donation basis.
Case Managers can provide information and coordination for the following:
- Home assistance with light housekeeping, laundry, lawn care, and minor home repairs.
- Prescription and health insurance assistance.
- Assessment of eligibility for federal, state, and county programs.
- Respite care options.
- Support groups, including caregiver support.
- Resources to address elder abuse and neglect.
- Financial and utility assistance programs.
- Recreational, socialization, and volunteer programs.
